Records Department Requests K-8

If you are a school district looking for student records.

Please send copy of signed parental release to:
Sara Moccio, Principal Clerk Student Records and Attendance
Email smoccio@haverhill-ps.org  or fax to 978-373-3815

If you are a parent looking for a copy of your child’s record who is still attending Haverhill Schools.

Please submit your request in writing to your child’s school and include the following information:

  • Students Name
  • Documents that you are requesting

If you are a parent looking for a copy of your child’s record who is no longer attending Haverhill Schools.

Please submit your request in writing to:

The request must include the following information:

  • Students Name
  • Grade and School last attended
  • Documents that you are requesting
  • Please indicate if you are looking for special education records.

Individual requesting copy of student’s records must present identification prior to release of documents.  Please note a student’s cumulative folder is destroyed 7 years after a student leaves the district.

Records Department Request – High School

If you are a school district looking for student records:

Schools requesting records should email their requests on school letterhead to hhsguidanceinfo@haverhill-ps.org

If you are a current or former student looking for records:

Former or current students may request a copy of their transcript by emailing the Guidance Department at: hhsguidanceinfo@haverhill-ps.org Please include the following information:

  • Name as it appeared when attended HHS
  • Year of Graduation
  • Date of Birth
  • Telephone number where you can be reached
  • Full Name and Address where transcript should be mailed

Please allow 2-3 business days for processing.
