Blue Cross Blue Shield

Haverhill Public Schools offers two health plans administered by Blue Cross Blue Shield.  Health insurance is effective on the first day of employment and you have 30 days from the date of hire to enroll in or waive.  The plan year runs from July 1, 2016  to June 30, 2017.  You may choose individual or family coverage.  The City of Haverhill pays 70% of the premium and the employee pays 30%.  Note: if you were hired prior to July 1, 2011, the City of Haverhill pays 75% of the premium and the employee pays 25%.  Please see the rate chart below.

HMO – Network Blue New England Deductible with Hospital Choice Cost Sharing   –  An HMO requires that you receive health services from providers within a network of physicians. You must choose a PCP and referrals are needed to visit a specialist.  Coverage is in-network only and limited to New England. Coverage does include preventive dental benefits for children under age 12.

PPO – Blue Care Elect Deductible with Hospital Choice Cost Sharing –  A PPO gives you the freedom and flexibility to visit any provider you choose; coverage is nationwide. You do not need to choose a PCP or receive referrals to visit a specialist.  The plan has in and out of network coverage  – though your costs are higher when you utilize an out of network provider.

Both plans offer retail and mail service prescription drug coverage through Express Scripts, a Telehealth benefit, the MYBLUE Member App which gives you access to an interactive ID card and the MYBLUE  portal to review your benefits, claims and to search for providers.   In addition, the following benefits are offered to encourage you to maintain or improve your overall health:

The following employee rates are in effect from July 1, 2016 – June 30, 2017.  The deductions are pre-tax, bi-weekly and deducted from 20 paychecks.
