Below are the Registration Forms for the 2026-2027 School Year. Complete the appropriate registration application below and upload ALL required documents. Enrollment is not complete until all the required documents are provided and validated.
NOTE: Enrollment for Moody Preschool is by Lottery Only, below is the lottery intake form. Please DO NOT complete the registration application until you are instructed to. If selected, the Moody preschool office will notify you to complete the registration application and a screening.
