MetLife - Preferred Dentist Program Plus (PDP)
This page will help you find a wealth of information to help you make the most of your dental insurance. MetLife offers 2 dental plan options with:
High - Option 1
Low - Option 2
You have ten (21) days from your first day of employment to complete and return the dental enrollment form to the Benefit Specialist. Your dental insurance will be effective the first day of the month following your first day of employment. The deductions are pre-tax, bi-weekly and deducted from 20 paychecks. Please note, premiums are paid one month in advance.

You will not receive a Dental ID card
You only need to inform your dentist that you are enrolled in the MetLife Preferred Dentist Program (group number is 0215113) and they can easily verify your enrollment information by contacting MetLife. You may print this MetLife City of Haverhill Dental ID Card to use or download the Met Life – Mobile App for a mobile ID card. In addition, both plans provide the MyBenefits Website to review your benefits, claims and to search for providers. When registering online the employer is “City of Haverhill." For questions you may contact the Met Life Dental Customer Service Center at 1-800-942-0854.
To find a dentist:
Visit the MetLife website and click find a dentist and enter zip code and choose the PDP Plus Network. Register with MetLife and you will be able to print out cards and view your benefits.
The plan year runs from July 1st to June 30th (deductible and calendar year maximum resets on January 1st).
