MetLife – Preferred Dentist Program Plus (PDP)
This page will help you find a wealth of information to help you make the most of your dental insurance. MetLife offers 2 dental plan options with:
High – Option 1
Low – Option 2
You have thirty (30) calendar days from your first day of employment to complete and return the dental enrollment form. The dental insurance will be effective the first day of the month following your first day of employment. The deductions are pre-tax, bi-weekly and deducted from 20 paychecks.
You will not receive a Dental ID card:
You only need to inform your dentist that you are enrolled in the MetLife Preferred Dentist Program (group number is 0215113) and they can easily verify your enrollment information by contacting MetLife. You may print this MetLife City of Haverhill Dental ID Card to use or download the Met Life – Mobile App for a mobile ID card. In addition, both plans provide the MyBenefits Website to review your benefits, claims and to search for providers. When registering online the employer is “City of Haverhill“. For questions you may contact the Met Life Dental Customer Service Center at 1-800-942-0854.
To find a dentist:
Visit the MetLife website https://www.metlife.com/ and click find a dentist and enter zip code and choose the PDP Plus Network.
The plan year runs from July 1, 2019 to June 30, 2020 (deductible and calendar year maximum resets on January 1).
Reminder: Dental for children under 12 is NOT included in any of the GIC health plans.
