    MetLife 

    MetLife is the administrator of the dental insurance.  Click here for the MetLife Dental – Welcome and Information Letter.

    We will continue to offer two dental options  – Option 1 High Plan (formerly Enhanced under Delta Dental) and Option 2 Low Plan (formerly Basic under Delta Dental).  Both plans utilize the same network – MetLife Preferred Dentist Program Plus (PDP Plus) and offer in and out of network coverage.  To find a dentist:  visit the MetLife wesbite https://www.metlife.com/ and click find a dentist and enter zip code and choose the PDP Plus Network.

    Option 1 High Plan–  Covers preventive, basic and major services, $1,750 per person calendar year maximum

    The plan year runs from July 1, 2019 to June 30, 2020 (deductible and calendar year maximum resets on January 1).

    You may choose individual, individual + 1 or family coverage.     The deductions are pre-tax, bi-weekly and deducted from 20 paychecks.

    Reminder: Dental for children under 12 is NOT included in any of the GIC health plans.

    You will not receive a Dental ID card.  You only need to inform your dentist that you are enrolled in the MetLife Preferred Dentist Program (group number is 0215113) and they can easily verify your enrollment information by contacting MetLife.  You may print this MetLife City of Haverhill Dental ID Card to use or download the Met Life – Mobile App for a mobile ID card.  In addition, both plans provide the MyBenefits Website  to review your benefits, claims and to search for providers.  When registering online the employer is “City of Haverhill“.   For questions you may contact the Met Life Dental Customer Service Center at 1-800-942-0854. 

    Also, as an employee you automatically receive the MetLife Vision Access Program – this is a discount program at a nationwide network of private practice ophthalmologists and optometrists.  Discounts included are on exams, lenses and frames – to receive your discount just give our program code of MET2020 to the participating MetLife private practice provider.  To learn more visit the website at www.metlife.com/mybenefits, call 1-888-GET-MET8 (1-888-438-6388) – say “Vision”, then select option 2 (MetLife VisionAccess Discount Program) or read the flyers below.
