As a retiree, you may continue your medical, dental and/or life insurance into retirement. Below is information regarding each benefit product Haverhill Public Schools offers its employees.

Please send a letter to Human Resources stating your intent to retire and the effective date.


Health Insurance:

Under age 65 at retirement:
As a retiree under age 65, you will remain enrolled in your current GIC health insurance plan.  The cost is the same however, you will now be billed monthly and the payment will be deducted from your pension (MTRS or City of Haverhill).
Complete and return the following form to Jennifer Schmidt at Central Office.

Over age 65 at retirement:
As a retiree over age 65, you are now eligible for Medicare Part B.  Medicare will become your primary insurance and the GIC Medicare supplement plan will be your secondary insurance.  You will now be billed monthly and the payment is deducted from your pension (MTRS or City of Haverhill).  The GIC will mail you an enrollment form and you will pick your GIC Medicare supplement plan and mail the form back to the GIC.  Complete and return the following form to Jennifer Schmidt at Central Office.  GIC Status Change Form

Turning 65 while in retirement:
As a current retiree turning 65, you are now eligible for Medicare Part B.  Medicare will become your primary insurance and the GIC Medicare supplement plan will be your secondary insurance.  You will continue to be billed monthly and the payment is deducted from your pension (MTRS or City of Haverhill).  The GIC will mail you an enrollment form and you will pick your GIC Medicare supplement plan and mail the form back to the GIC.  City Human Resources  administer’s health insurance for all retirees.  Contact City HR at 978-374-2357.

When to enroll in Medicare:

Everyone MUST enroll in Medicare Part A upon turning age 65 – it does not matter if you are actively employed or already retired.

Medicare Part A:
Enroll in Medicare Part A three (3) months prior to turning 65 with Social Security.
Three ways to enroll:
1. Online at SocialSecurity.gov
2. Calling Social Security at 1-800-772-1213 Monday-Friday from 7:00am-7:00pm
3. In-person at your local Social Security office

Medicare Part B:
If you retire before turning age 65:
Enroll in Medicare Part B three (3) months prior to turning age 65 while in retirement.
If you retire after turning age 65:
Enroll in Medicare Part B three (3) months prior to your effective retirement date.

Enroll by completing and returning the following forms to your local Social Security Office:
1. Employee Form CMS-40B
2. Employer Form CMS-L564

You will have three (3) ID cards – one Medicare card, one GIC health plan card and one prescription drug card.


Dental Insurance:

As a retiree, you will remain enrolled in your current dental plan.  MetLife will transfer you to the retiree group.  Your Group number remains the same.  The cost is the same however, you will now be billed monthly and the payment will be deducted from your pension (MTRS or City of Haverhill).


Boston Mutual Life Insurance:

Basic – Div 2
The Basic Life policy of $5,000 will continue into retirement.  The cost is the same however, you will now be billed on a monthly basis ($5.40) and the payment will be deducted from your pension check (MTRS or City of Haverhill).
Complete the following Beneficiary Form and return to Jennifer Schmidt at Central Office: (Original needed – please drop off in person or send via interoffice mail – no scanning)

Optional  – Div 5
The Optional Life policy will continue into retirement.  You will now be billed on a monthly basis and the payment will be deducted from your pension check (MTRS or City of Haverhill).

Voluntary – Div 8
The Voluntary Life policy cannot carry into retirement.  If you wish to continue coverage, you will be billed directly at your home address. Right to Convert Provisions

AllState:

As a retiree, these insurance products are portable and AllState will be notified to direct bill you at home.  Please contact Life Plus Insurance Agency at 781-837-9222. with any questions you may have.


Colonial Life:

As a retiree, these insurance products are portable and Colonial Life will be notified to direct bill you at home.  Please contact Colonial Life at 401-596-1510 with any questions you may have.


Unified Life:

As a retiree, these insurance products are portable and Colonial Life will be notified to direct bill you at home.  Please contact Unified Life Insurance Company directly at 800-237-4463 for information regarding your life insurance policy as it does have cash value.


403(b) :

PenServ will be notified to change your status to retired as of your effective retirement date.  Please contact PenServ at 800-849-4001 if you wish to submit an initial withdrawal or transfer of your 403(b) funds.
