H1N1 - Public Vaccination on Thursday January 21, 2010 scheduled for the High School Students.
Haverhill School Health services along with the Haverhill Health Department will be at Haverhill High School on Thursday, January 21 from 8:30-11:30 to offer the H1N1 vaccine to students. In order to receive the vaccine, students must return a signed parent consent form prior to Thursday. Consent forms will be available in the main office at the school or on the Haverhill Public Schools web site-there are two pages that need to be completed and returned.

Forms: Can be downloaded to complete in advance, or will also be available in the Main Office of the High School.
