Basic Life Insurance and Other Voluntary Benefits Summary with LifePlus Insurance Agency.

Boston Mutual Life Insurance Company

Basic Life Insurance – A $5,000 term life insurance policy, which also includes accidental death and dismemberment, is available for the employee only.  The City of Haverhill pays 50% of the premium and the employee pays 50%.  The current employee premium is $3.25 per paycheck.

Voluntary Life Insurance – If you enroll in the basic life insurance, you may elect to enroll in supplemental term life insurance, which includes accidental death and dismemberment, for yourself, your spouse and child(ren). (Employee must elect coverage in order to cover spouse and/or child(ren)). No medical questions asked for coverage at or under the Guaranteed Issue Amount.  The employee pays 100% of the premium. The rates for employee and spouse are based on age (5 year age bands) and how much life insurance you elect.  The rate for child(ren) is a flat rate.

You must enroll within 10 days of your first day of employment.  Complete and return the enrollment form to the Benefit Specialist.  The insurance is effective the first day of the month following one month’s service.  Premiums are deducted from 20 paychecks.

Cancer Expense Plan – You may choose individual or family policies. The policy includes an annual wellness benefits.

Critical Illness Insurance – You may choose individual or family policies. The policy offers flexible benefit levels and includes an annual wellness benefit.

Disability Insurance  – This plan covers both sickness and accidents and offers flexible elimination and benefit periods.

Permanent Life Insurance – A whole life (aka universal life) insurance policy is available for yourself, your spouse and child(ren).  The insurance has flexible level premiums, guaranteed cash values and guaranteed death benefits.  Rates are based on age and gender.

Critical Illness Insurance – helps with expenses if you’re diagnosed with a covered specified disease, surgery or cancer.   The insurance includes a $50.00 per covered person, per calendar year health screening benefit which can help pay for health and wellness tests you have each year. These benefits are available for you, your spouse and dependent children.  Claims may be submitted online via ColonialLife.com, fax or mail.   Benefits are paid directly to employees.   You own this policy with no increase in premium or change in plan design and can take the coverage with you if you change jobs or retire.
